Skilled Swing BedSwing Bed is a Medicare program designed to provide additional skilled care to those needing extra time to heal before returning home. We are committed to a goal of helping patients who are recovering from an acute hospitalization return to a normal level of functioning.
Our Swing Bed program allows patients to move from acute care to skilled rehabilitation at Tri Valley Health System. This ensures continuity of care for patients needing nursing care by keeping them in the same facility and room with the same provider, nurses and therapists. Qualifying individuals need daily skilled nursing or skilled rehabilitation services which can only be provided in a skilled nursing facility such as Tri Valley Health System. |
Who can benefit from Swing Bed care?
Any patient who does not need acute care but is not ready to go home can benefit from Swing Bed care. Below are examples of patients who may need this service:
- Recovery from major surgery, joint replacement, vascular or abdominal procedures
- Extended hospitalization
- Recovery from a major accident or stroke
- Pain Management
- Intensive Wound Care
- Nutritional Therapy
- IV Therapy
- Rehabilitation Therapy: Physical, Occupational, Speech or Respiratory
How do I qualify and pay for Swing Bed care?
Medicare will cover skilled swing bed if:
- You have Medicare Part A and have available days left in your benefit period.
- You have a qualifying hospital stay (a 3-consecutive day acute care stay).
- Your provider has determined that you need daily skilled care. Care must be given by, or under, the direct supervision of skilled nursing or rehabilitation staff.
